This form is used by Theme Surgeons to obtain information necessary to services related to hosting, or hosting changes, including moving/migrating a WordPress site from hosting service to another for design partners. To avoid delays, please make sure that you have verified that all logins we need actually do work.
We will contact you with regard to any other questions we have once we have verified the current hosting arrangements for clients who are moving sites from an external provider to Theme Surgeons/Webfaction hosting plans.
